What is CT WiZ?

CT WiZ is Connecticut's statewide immunization information system. It is a centralized database that tracks vaccination records for residents. The system was established by the Connecticut Department of Public Health to collect, manage, and maintain immunization data.

Key Features of CT WiZ
  1. Centralized Record-Keeping:
    • CT WiZ stores vaccination records for children and adults across the state.
  2. Reporting Tool for Providers:
    • Healthcare providers are required to report immunizations for individuals under 18. Reporting for adults is optional.
  3. Data Sharing with Schools and Public Health Officials:
    • CT WiZ allows schools and public health agencies to verify immunization status for compliance with state requirements.
  4. Parental Access to Records:
    • Parents and guardians can request access to their child's immunization records through the system.

How to Opt Out of CT WiZ

Please note you MUST send individual letters for every member of your family, you cannot send one letter for the entire family. CTRAMM recommends sending this letter certified mail as there is no form of confirmation that you have been removed from the system.
To opt out: Send a letter if you want to opt out of CT WiZ. Make sure you include the full legal name, date of birth, and address of yourself or your child (if under age 18). Send the letter to us by mail or fax:

Do NOT send your request by email. Routine email messages are not secure.
After CT Wiz receives your request, CT WiZ will no longer update or make available your vaccination record per State law.
